Chart Review Database

Important Note: 
It has been recently found that there is an error in the database applicable to sites collecting data using the units mg/dl. Most sites collect lactate using mmol/l as the unit of measure. This is true for instance in nearly all sites in the United States.

The error results in your data showing a poorer performance than you should expect to the CVP and ScVO2 measures in particular. Since 4 mmol/L is equivalent to 36 mg/dl, the error results in sites using mg/dl as the unit of measure more frequently being scored as failing to check CVP and ScVO2 when indicated.

In truth, the impact is likely to be minimal on your performance. However, it is important for your own results at home to complete the upgrade.


To upgrade your database to version 4:

  1. Unzip this file http://www.survivingsepsis.org/files/V4FrontEnd.zip.
    The file inside is named "SurvivingSepsis."  (The full extension is
    SurvivingSepsis.mdb" if your computer shows the extension by default.)
  2. Use the Search feature in Windows (Start --> Search) to locate the folder
    named SepsisDB. The most likely path for this folder is "C:\SepsisDB"
    but if you have installed on a network drive may be another drive such
    as "S:\SepsisDB."
  3. Drag the unzipped file SurvivingSepsis into the folder SepsisDB wherever it is located.
  4. The computer will ask you if you want to overwrite the existing file. Agree to overwrite the file.
  5. Next, goto your desktop icon and open the sepsis database.
  6. The database will notify you, "The links are broken, do you want to re-establish the links?" Agree to re-establish the links.
  7. The database should open normally and all of your data will be preserved.

Once you have upgraded your database to version 4, re-transmit your data to the Surviving Sepsis Campaign. We need you to do this to ensure we have accurate data. We are able to manipulate your data, if in error, on our end, however re-transmitting the data avoids this step.

You may transmit data by sending your data by email. This is the most reliable means to transmit data.

· On the main screen of the database, select the "Export and Transmit Data" button. 
· Note the path in the field in the middle of the subsequent blue screen. By default this is probably "C:\SepsisDB\Export\" 
· Be sure the box "Anonymize Data" has a check mark in it. If not, select the box to place a check-mark in it.
· Select the "Export Data" button. Your data will then dump into the folder identified in the path above(probably "C:\SepsisDB\Export\") in 3 files.
· Go to the path where you data is stored. You will find 3 text files (chart.txt, abx.txt, chartdefaults.txt).
· Attach all 3 files to an email and send the email to
ssc@survivingsepsis.org.

You will receive a confirmatory email from the "Out of Office" feature in Outlook that confirms your email was received.
